Application Instructions
The Carlson Cares Fund was created as a last resort to help employees, their spouses or eligible dependents who are facing financial hardship because of a qualified incident (see Eligibility Requirements page for more details about qualified incidents).
An employee can only receive financial assistance once within a 12 month period. In the case of a denied application, the employee is eligible to submit a new application (for a different incident) after six months. In the case of an employee’s death, eligible dependents may apply.
The maximum amount available for each incident is $5,000. Grants may be awarded to help pay for expenses directly related to the qualified incident. In the case of lost wages caused by the qualified incident, the Fund may be able to provide financial assistance for regular monthly bills up to the amount lost in wages.
Grants will not be awarded for expenses including, but not limited to: legal fees, insured property losses, insurable medical conditions, bills unrelated to the qualified incident, bills with service dates over 150 days prior to the application date, non-essential items or services, etc. In some cases, The Carlson Cares Fund cannot pay for items due to lack of home owners or medical insurance. Please call +1 (612) 672-8671 to discuss.
Applications are reviewed by The Minneapolis Foundation and will be treated in a confidential manner; however non-identifying statistical information will be reported to the company on a periodic basis.
Fully complete applications will be reviewed within five (5) business days. Submitting an incomplete application will delay review. Applications must be submitted within 150 days of the qualifying incident. Employees are encouraged to keep a complete copy of the application for their personal records and to have on hand should we have any questions.
Take your time when filling out the application. Be sure to answer every question and attach every required document. If you have any questions, please call.
If the application is approved, the Carlson Cares Fund will issue the grant in the form of a check(s) payable to the vendor(s) to whom the employee owes payment(s). The Carlson Cares Fund does not issue checks to employees.
